Progress Photo Consent

Important: This is a practical policy template for the current app. Andrea Wellness Co. should have a licensed attorney review it for its business location, services, and applicable state laws before a broad public launch.

Optional feature

Progress photos are completely optional. Declining this consent prevents photo uploads but does not prevent use of the rest of the Rooted In W3llness membership.

What I authorize

By checking the consent box and saving a progress photo, I authorize Andrea Wellness Co. to collect, store, and display my submitted progress photographs and related date or note inside my private Rooted account for wellness coaching, progress review, account support, and service security.

Who can view the photos

Photos may be viewed by me and authorized Rooted admins or coaches who need access to provide the service. They will not be posted to Crew automatically. They will not be used in advertising, testimonials, social media, public before-and-after content, or promotional materials without a separate written authorization from me.

How photos are handled

The app resizes photos and removes location metadata before upload.
Photos are stored in private cloud storage and accessed through restricted, time-limited links.
Photos may reveal sensitive information about my body, health, surroundings, or identity even after metadata removal.
No online storage method can eliminate all risk of unauthorized access.

My choices

I may delete a photo set through the app. I may withdraw consent for future uploads by contacting Rooted. Withdrawal does not make prior authorized processing unlawful and may not remove limited security, backup, or legal records immediately. Rooted will not require progress photos as a condition of membership unless a separate service agreement clearly says otherwise.

Not medical imaging

Progress photos are not medical records, diagnostic images, or a substitute for evaluation by a qualified health professional. Coaches should not use them to diagnose or treat a medical condition.

Acknowledgment

By accepting version 2026-08-27, I confirm that I am at least 18, have read this consent, understand the privacy risks and permitted uses, had the opportunity to ask questions, and voluntarily agree to the handling described above.
